Parents, Coaches, and Athletic Directors,
All Shelby County Schools student-athletes must have a completed and signed pre-participation physical (dated after April 15, signed by FNP, MD, or PA, with complete medical/contact information, consent for sports participation and consent to treat, and signed by parent or guardian), Tennessee Sudden Cardiac Arrest Act signature form, and Tennessee Sports Concussion Law signature form on file at the school prior to tryouts or participation in practice.
